Hospital Aftercare Service

Our aim is to help you overcome any immediate problems you may have on returning home from hospital. This service provides free, practical help and emotional support for older people, who may otherwise be left isolated and vulnerable after leaving hospital.

Who can we help?

We can offer help if you live in the Blackpool area and you are over the age of 18.

Do you have a choice?

Yes. At all stages, the choice is yours. We offer our services to you. You don't have to accept our help. If you do, we will discuss the type of help you may need. You can ask us to stop at any time.

Who will provide you help?

Age Concern Blackpool has trained staff and volunteers specifically to help and support you.

How much will it cost?

Nothing. The help is free to all who receive it.

For how long will i receive help?

It will normally be once a week for a few weeks only, depending on your needs.

Will it affect my entitlement to statutory services or benefits?

No. The service is designed to complement any statutory services that you may be receiving and will not affect your entitlement to benefits.
